At Grace Neighborhood Academy, we make enrollment simple and accessible for all families. Whether you are enrolling your child in our toddler program, preschool, before and after school care, or summer camp, we have all the necessary forms and information available below.

Application Forms

Please download and complete the appropriate forms for your child’s program:

Enrollment Applications

  • 2024 PreK Counts/Head Start Application – For children enrolling in our PreK Counts or Head Start programs
  • GNA Toddler Application – For children ages 1-2 years
  • GNA School-Age Application – For children ages 5-12 years in our before/after school care
  • Summer Camp Application – For summer program enrollment

Head Start and Pre-K Counts families can also apply online at: Philadelphia School District Child Care Portal

Required Health Forms

  • Child Physical Form – Required for all enrollments
  • Child Dental Form – Required for all enrollments
  • Administration for Medication – If your child requires medication during care

Additional Required Forms

  • CBS Food Application – For meal program participation
  • Emergency Contact Form – Required for all families
  • Tuition Agreement – Payment terms and policies
